In a US hospital, a patient in critical condition is wheeled into the emergency room. The doctor orders various tests and the report dictated by the physician is sent to Pune, to a Medical Transcription (MT) company. An hour later, back in the US, the doctor starts the surgery aided by the transcript from the Pune company.

After hitting a low in 2002, the MT industry in the country started showing signs of revival in 2006 and has now come of age with not even the global economic slowdown affecting the workflow from the US.

Many MT companies in Pune are in the expansion mode and are on a huge hiring spree.

“There has been no effect of the slowdown on our business.In fact the volume of business has reached a high and we are planning to recruit 100 hands soon to meet the growing volume. We acquired a US MT firm Medical Bazar in 2006 and are now finalising a joint venture with Lotus Logistic in the US. We will hold 75 per cent stake there,” said Sumanta Gupta, Managing Director, Crossover Medical Technology Inc that caters to over 80 clinics in the US. Crossover is also starting its centre in Bangalore, Hyderabad and Alappuzha in Kerala.

Another Pune-based MT firm, Sharp Hearing Technologies, is also in the hiring mode.

“We are finalising a contract with a big hospital in California with 1,200 doctors. As soon as the deal is done, we will recruit 100 more employees,” said Yogesh Bhosle, Managing Director, Sharp Hearing Technologies.

According to him, after it started reviving in 2006, the MT industry has matured and only truly healthy ones survived.

“MT training institutes were mushrooming before it hit a low. Many closed down or moved to other jobs. Now, the qualified workforce and industry maturity are some of the main reasons that draw greater amount of work from the US,” said Bhosle, who plans to employ 90 out of the fresh 100 it is recruiting as home-based transcriptioners.

According to A Chetan of Bangalore-based Lloyds Solutions Pvt Ltd, the scene is no different in Bangalore. “The work volume for MT firms in general has picked up. The reason attributed to it is the quality of delivery that the Indian firms offer. As for us we have 6 major hospitals and 4 clinics as clients. We have an employment strength of 200 and plan to hire more as per the increase in the volume of work,” he said.

As per a recent strategic report of NASSCOM, the MT segment in India that earns an annual revenue aggregate of approximately $ 220-240 million and the MT industry will be worth Rs 40 billion by 2010 and could employ as many as 50,000 people.

The work off-shored was expected to be in the region of $ 860 million in 2010, of which India is expected to capture $ 647 million.

According to MTIA, currently 8% of physician-patient encounters are documented via electronic medical records; by comparison, more than 80% employ the dictation-transcription process for this purpose. MTIA estimates that over 1.2 billion patient notes are dictated and transcribed annually, and currently the majority of those records cannot be imported into electronic medical records in digitally useful formats.

Working with Verizon Business, the consortium plans to offer an industry solution for medical transcription service providers to exchange medical records in digitally useful form. Verizon Business expects the platform to be available later this year.

The killer application would be to be able to use Google Voice on a mobile phone, and avoid using minutes from your monthly plan. Accordingly, Google submitted a Google Voice app to Apple for the iPhone, but Apple is not co-operating. Today I got a question from reader named Laura. She asked:

Experience also has to be recent. Although I have about 5 years of experience, it is all over 9 years ago so I would have to start over as far as gaining experience. So these tips are useful to experienced transcriptionists as well who haven’t done transcription work recently.
Tip 1: Graduate from a program respected by companies that hire transcriptionsts to work remotely or offers job placement

I’m assuming she means how to get a medical transcription job with no experience. This is a very common question for new transcriptionists. If you haven’t graduated from any of the the “Big Three” medical transcription programs (Career Step, Andrews School, and M-Tec) and your school doesn’t offer job placement - it can be very difficult to get your foot in the door to work at home. If your goal is to work at home, the least experience required I’ve seen from online companies is one year. Even then, be prepared to be baptized by fire as you may get most of the harder jobs (hard to understand foreign accents, poor recordings) and be paid a lower rate at first.
Tip 2: Offer a discounted rate or free. Work as an intern or a volunteer

When I became a transcriptionist, I had not gone to school for it, so I had no education or experience. The best I had to offer was knowledge of medical terminology from a medical office program I had completed.

I approached a local medical transcription company here in town and offered to work for them on the dreaded graveyard shift for free for a couple months as an intern while they taught me. We both agreed if I did well enough, they would hire me. They hired me after a month (and I got re-introduced to the daylight!). According to a patent filing, Apple is working on speech-to-text technology for its iPhone and iPod product lines. Speech recognition could be the holy grail for data entry and retrieval on mobile devices, especially as they continue to shrink in size.

The Baltimore Sun found the patent and has included a diagram of how the system would work when composing an email.

There is a lot of engineering speak in the filing, but I could decipher a few tidbits of info - that and I've seen this stuff on Star Trek so I know how it is supposed to work. It seems the speech recognition module they are working on would be able to not only handle text but non-speech data as well, such as punctuation.

To varying degrees, this has been tried before on mobile devices. The most rudimentary are the voice snippets you can record into your phone for a few of your favorite contacts. One of the better speech tools for phones is byMicrosoft (NSDQ: MSFT) and called Voice Command. It is really pattern recognition. You can say "Call Sally Jones at work" and it will search through your contacts and find a name that matches what your digitized voice said and dials the number. You don't have to train it or record her name before you can use it. You can also ask it the time, battery level, signal strength, upcoming appointments and more. It is rather limiting though and there is no way to compose an email with it or tell it to do anything outside of dozen or so tasks it was written for.

I recall one demo by Bill Gates a few years ago where he spoke into a Pocket PC (that is what they were called way back when) and got nearly flawless text recognition out of it, but the trick there was the voice data was converted to digital then sent via wireless to a powerful server which did the heavy lifting. It returned the text to the screen. In the day's GPRS networks, it just wasn't feasible, which is why he was using WiFi. Today with 3G networks, it is more realistic, but you have the issue of who is going to pay for the server to potentially service hundreds of thousands of voices simultaneously?

Hopefully, maybe even those MTs who are techno-phobic will find some of the topics enlightening, stimulating and/or of value in making career decisions. But maybe not. To challenge that hope, I've started off with everyone's favorite technology: speech recognition. If you want to really stimulate a transcriptionist, just say "speech recognition." Or, better yet, assert that "speech recognition will forever change the process of converting physicians' thoughts and utterances into text." Then run for cover.

Every transcriptionist out there has heard some form of that assertion. Their reactions range from dismissal to fear to anger. So what's the truth? What does the future hold? Well, at some point in the future, there will be no medical transcription. Physicians will dictate into a PC or portable device; their speech will be converted to text; and the dictator will make any necessary corrections to finalize the report. No transcription expense. No transcription delay. But that future is at least 3 years off. Just kidding. It's way more than that. However, there is a future closer than that, related to speech recognition, which has some major implications for this industry.

Doctors hate doing anything that they believe is below their stature or slows down their ability to generate revenue. So we will not see "front-end" recognition-where they correct their own mistakes as described above-in most environments for many years. But there's a new game in town. More than 60 years ago, medical transcription was developed as a method to assist physicians and medical facilities with accurate recording and filing of patient information related to treatment. In the 1970s and 1980s, some medical groups were beginning to see the need for more training in the area of medical transcription. This led to formal training programs that were designed to incorporate medical terminology, anatomy and typing to more accurately equip persons in this field. This was a tremendous success and medical transcription became a profession in demand.

And just as we were getting comfortable in our new skins as MTs, we entered into the age of computers. With this new age, medical transcription became an organized business. Because of the high cost involved in this process, medical transcription service organizations (MTSOs) sprang up to meet the needs of the industry. So you've gone through training as a medical transcriptionist and you're ready to work. Surely someone will hire you!

This may be the hardest part if your school does not offer job placement assistance or if you studied on your own. Not all or even many companies hire inexperienced medical transcriptionists. It can pay, however, to check with companies whose listed requirements you do not meet due to lack of experience. Some may make exceptions if you can pass their test.

Testing is a quick way for a company to find out what your skill level is. If the company is local, they may test you at their location; distant companies may send out a test or have you test online. No matter how the test is performed, don't rush, and make sure to check your work.

A good school will help you with finding companies willing to test and hire you.

You will want to have some idea as to the schedule you will be willing to work. I would suggest being available for at least 20 hours per week. Some companies may require that you stick to a schedule, so be realistic about the hours you can work. Don't assume that any children you may have will cooperate or that naps will always be a particular length. You'll have an advantage if they are in school, but otherwise, be ready to work at night or early in the morning when the children are asleep. You will need peace and quiet to be able to understand some of the doctors.

Be careful when you choose an employer, as there are scam companies in this field, just as in many other work at home opportunities. Some companies out there claim to hire completely inexperienced people and train them as medical transcriptionists, but so far I have not heard of one that was not a scam. The catch is that you can never meet their standards to become a paid employee. Don't fall for it.

Fortunately, there are plenty of good companies out there. With outsourcing to other countries it can be a little rougher than it used to be, but the jobs are still out there. If your school offers help in landing that first job, take their help. Some only include it with their more expensive plans, so you want to think ahead even before you begin training.

All is not lost if you don't have help finding that first job. Research medical transcription employers and ask on medical transcription forums about companies willing to hire new graduates. Check with local doctors, dentists, chiropractors, etc. The jobs are out there, it just takes a little effort to find them.

Medical Transcription and Voice Recognition

No doubt, you’ve heard different opinions and views on this subject. If you’re a medical transcriptionist you probably can’t understand how voice recognition can take over for a trained qualified medical transcriptionist. And you’re not alone.

Let’s dig a bit deeper into voice recognition and medical transcription.

As you already know, doctors are busy people. This is never more obvious then when they’re dictating their notes. It’s understandable they’re busy, and as their MT, I can surely forgive them but will the latest voice recognition software be as forgiving as me?

Not likely.

As a transcriptionist you will have typed through background noise, patients moaning, doctors eating their lunch, personal conversations (oops they forgot the recorder was on) and other incomprehensible noise. Not to mention, ESL doctors with heavy accents and very tired ER doctors after a long shift!

At this time there is no voice recognition software which can handle this type of voice recognition. It is impossible for the software to determine actual speech from mistakes in conversation, background noise, heavy accents, etc.

So what does this mean for our future?

I wouldn’t worry about voice recognition. Rumors of MTs beign out of r a job have been around long before I became an MT. Eight years later, there are still no real advances in this field.

Can voice recognition ever replace transcriptionists?

Sure it can.

If a doctor is willing to sit down and take the sufficient time to train his voice recognition software to recognize his voice and speech patterns (this takes time and is not done automatically), yes it is possible.

If the doctor thereafter dictates very clearly, using proper punctuation in his speech (stopping for periods, pausing for commas) without any background noise or interruptions. Yes, it is possible.

Will the document be 100% accurate? No.

Remember medical records have to be in compliance with a number of very strict regulations. Most doctors, will not trust voice recognition enough to send these records through without at least a quick glance through.

Even under the best dictating circumstances the report will still need to be proofread and edited. So, yes under the “perfect” circumstances, voice recognition can replace a transcriptionist.

Is it likely? Not unless every physician out there is willing to take the time, energy and ongoing effort to train their voice recognition software and maintain a certain standard of dictation.

I don’t see that happening any time soon. Doctors are busy people, remember? ;)

If anything, us MTs should embrace voice recognition and use it as a tool to help us in our MT careers. If applied properly, it can be a time-saving tool. So why not use it for our purposes?

As with any business to stay ahead of the game you have to adapt to change and technology. Learn how to use it to your advantage instead of being frightened by it. That’s the only way to stay ahead of the competition…. Voice recognition or otherwise.

Usually, they will ask for writing or transcription samples to ensure you are qualified, and then begin regularly sending the recorded media to be copied down. The job is extremely simple, and is ideal for anyone looking for a good way to work at home. Listen to the recorded media, write down exactly what is said and format it a way that is easy to read. Of course, it isn’t always as simple as that. Some work requires a good deal of specialization which, if you are looking for an easy way to work from home, may not be something you have. Transcription that is extremely technical, in the legal or medical fields, or esoteric in another way, may be rather difficult to interpret for the layperson doing the work at home. However, for someone who has a background in that area, who perhaps works as a paralegal during the day, such problems shouldn’t be difficult. In fact, having that degree of skill, especially in a field typically filled with people who work from home may give you a leg up on the competition.

Technical sophistication and esoteric information may not be the only problem faced when doing transcription work from home. A far more likely scenario is being given a recording that is simply inaudible or impossible to decipher. For whatever reason, a microphone or recording malfunction, or even just a quiet speaker, some recordings may make it nearly impossible to transcribe. However, there are a few routes around even this. For short, hard to hear gaps, using the surrounding context is likely to give a clue to the meaning of the indecipherable portion. If only one word is difficult to hear, it can probably be inferred from the surrounding sentence, especially if it is a common one.

On the other hand, some recording faced when doing transcription work at home may be far larger. For longer and more intransigent gaps, it may be necessary to attempt to enhance the quality of the recording using common sound editing software. A few tricks may be necessary to remove static and other problems.

Overall, transcription can be an excellent way to earn a little money doing work from home. It may even be possible to solely rely on it, turning the work into a small business of sorts.

Medical Transcription - Online Versus Classroom Training

If you've considered a career in medical transcription, you've probably already spent at least some time looking into the study options. And there are some serious decisions to be made when it comes to those training options. Should you study medical transcription in a classroom? Or would an online or correspondence course be better? It depends on your personality, your location and your needs.

Start by looking into schools in your area that offer medical transcription in a classroom setting. The sheer fact of your location in relation to any schools you'd be interested in attending may very well make the decision for you. While location is sometimes the deciding factor, don't let yourself be talked into taking what you consider to be second-rate medical transcription training just because it happens to be nearby. You can certainly get quality training from an online or correspondence school if there's nothing close to your location.

Your own personality - and limitations - should also play a role in your choice of training options. Do you work well without strict supervision and deadlines? Can you set goals for yourself and follow through? If your answer to those questions isn't a resounding "yes," you may not do well with online or correspondence training. There's a big difference between sitting in a classroom every day (or once a week) and being turned loose to do your own work. In a classroom, you're held responsible for keeping pace with the class. A dial-up connection with a slow computer may simply not be able to present the class in "real time." The lag can be annoying or even embarrassing, and you may find yourself losing out on some important points.

Regardless of what kind of training you choose, be wary of those offering claims that seem simply too good to be true. No one can guarantee that you'll find a job the week you complete your training and a school that offers those kinds of promises may very well be exaggerating other points of their training as well. Take time to be sure that any training facility you choose is reputable and that your hard work will end with training that will make you qualified to work in the medical transcription field.

Interview transcription - the importance of it

Recorded taped interviews and transcriptions are two sides of the same coin, many believe that a transcription is a back office task, but in actual it is an effective document of authenticity, in which every spoken word of the interview is documented in precise detail.

There are two forms of transcriptions of an Interview, one which is realistic where the pauses or repetitions are included and the other where it is not, meaning the irrelevant things are edited and corrections in grammar are effectively done.

Interviews may be between a celebrity and a moderator, a policeman and a criminal, an employer and an employee, a marketing team conducting surveys over the telephone; basically it is an interaction between two or more people.

And many people would be wondering, why would anyone require a transcript of an interview, since the audio of which is already available, consider this and review the importance of a transcript. There was a police officer who was in the process of filing criminal charges against a criminal, and based on the testimony of two key witnesses, the interrogation reached a stage where the criminal had no choice but to accept to the crime he had committed, and this confession of the accused was recorded, and to give authenticity to the confession, if a transcript or a text confession signed by the criminal was not made available to the court, the entire effort of the police officer in getting the conviction in the court will not materialize.

The other instance that also will need to be mentioned here, is of a student in the process of presenting research material on Oral History, the audio interviews based on his research was of 16 hours in length, and after editing the portions not relevant to the research a transcriber could format the transcript to seven pages only, and thanks to the transcriptions, it made reading them more interesting rather than listening to them for 16 hours.

The conclusion one can draw from these two instances is never make compromises on getting transcripts of the audio recordings, you never know how important they will matter at some stage.

A spoken word is not a sparrow. Once it flies out, you can't catch it.

Medical Transcription sector to boom despite recession: report

Despite global economic downtrend, the Medical Transcription (MT) industry is looking for a further growth buoyed by a report of the NASSCOM stating that recession has not hit the healthcare industry.

Reverse is the trend in medical transcription outsourcing in the US, which is witnessing agonising attrition in large scale in IT, financial, manufacturing and other sectors.

As per a NASSCOM report, the MT industry will be worth Rs 40 billion by 2010 and could employ as many as 50,000 people. It predicts that this industry is here to stay for at least 10 years.

It estimated the size of the US MT industry, which is in the range of around $ 12 million in 2005, would reach $ 16.8 billion by 2010. The work offshored was expected to be in the region of $ 860 million in 2010, of which India is expected to capture $ 647 million. India remained a preferred offshore destination primarily due to availability of manpower and industry maturity.

The report also expected the employee strength of 18,000 in 2006 in India-based MT vendors would reach 52,000 by 2010. For an industry that witnessed too much crowding, too fast in India, a report conducted by US-based Stevens International Consulting expected the value of MT outsourcing in the US to double by 2005 to $ 4 billion. India could take as much as two-thirds of that increase, providing work to 45,000 transcribers.

The Dark Side of Medical Transcription

The Tale of the Rubber Checks
Joan Edwards (not her real name), like Trudy Schaefer Looney, got into transcription on a whim 14 years ago. "It was sort of by fluke," she said. "I basically trained myself. I started out with a doctor and his wife. It was a brand new practice, and we sort of trained each other."

Edwards worked there for 4 years until the clinic closed due to lack of business. She moved on from there, getting her own accounts and working directly for clinics. Everything in her MT world was fine until she started working for a medical transcription service organization (MTSO) 2 years ago. The warning signs were there. The company said it would get direct deposit for its 120 MTs nationwide. It didn't. The promised health care coverage never showed up. A notice was sent out by the company saying that 10 oncology MTs quit, all in the same day. And MTs were forbidden by the company from contacting each other, even if they worked on the same accounts. Also, a lot of times Edwards and other MTs saw discrepancies in the line counts. They thought they typed more lines than they were paid for, but the only way to prove it would be to purchase the company's own line counting software, which would cost a hefty $300.

Medical Transcription Training

Employers will almost exclusively hire transcriptionists who have completed some kind of formal training in the area or who have experience. There are many ways to obtain such training, but for most a community college or vocational school will be the best option, especially if the training includes on-the-job experience. Do not worry about whether or not they offer the opportunity to earn the Certified Medical Transcriptionist designation. This is not a required certification in general, and in fact, most transcriptionists need a couple of years' experience before they can pass the test to gain this designation. I am not a CMT, but I was able to find a job.

Courses typically take several months to two years to complete, depending on the school chosen. Many vocational schools include the cost of books and course materials in their tuition, which may be helpful after you have landed a job. If you already work in a medical field, and simply want to switch to working from home, you may only need to learn transcription, as you may already have the vocabulary you need, but think carefully, as a refresher course may not be an entirely bad thing either.

It is also possible to take transcription courses at home. This option is more flexible but may be more challenging in other ways. Your selection of a course of study should depend on how you will best learn what you need to know.

You will need an assortment of reference books. Some you may recieve with your training, but you may find you need others. See the equipment page for suggestions.

College Courses

College courses are best for most people. Having an instructor to help you through the difficult parts and to keep you working on a schedule can be a great benefit. Many schools also offer job placement assistance, which may not be available through home study courses.

Community Colleges

Perhaps the cheapest method of instructor-led training you can get, but it may not be quick. A quick look at a local community shows a 22-25 unit requirement for certification in Medical Transcription. This would require a minimum of two semesters' work or more, depending on prerequisites and whether you attend part- or full-time.

Costs vary by what state you live in. Check with your local school.

Vocational Schools

Vocational schools cost more than community colleges, typically into the thousands, but may offer more flexiblity when it comes to the timing of your class. This may not be a quick solution either. With a quick look at a local vocational school's webpage, I found that its Medical Transcription training program takes about 28 weeks of classroom time and four weeks of an externship. Check to see if textbooks and other supplies are included in the cost.

Home Study

Home study courses offer the most flexiblity in many ways, but they offer challenges as well. You must be capable of working on your own. On the other hand, if you want to work at home, that's a skill you need. You'll need special equipment for this option in many cases, such as a foot pedal.

In the time I've spent researching medical transcription schools, three names keep coming up as the ones that employers actually hire from regularly: Career Step, M-Tec, and Andrews School of Medical Transcription. Of them, CareerStep is the most affordable. The others have a still better repuation with employers, but CareerStep graduates are plenty well enough trained to find employment. My former employer, Medquist, requires that you take the Gold level or above from CareerStep (last I heard, anyhow), which is my recommendation as well. Expect to take a minimum of 6 months, often 9 months or more, to complete a course.

Which option is best? That depends in large part on how you best learn and what is available in your area. There's no one definite right answer that will meet everyone's needs.

RP to benefit from US plan on medical IT

MANILA, Philippines— The decision of incoming US President Barack Obama to spend billions of dollars to build a fully integrated health information technology system for American hospitals and doctors’ offices will generate new business for Philippine business process outsourcing companies, according to a domestic labor group.

The Trade Union Congress of the Philippines (TUCP) said it was counting on the conversion to electronic records by US hospitals and doctors to boost local outsourced medical transcription and data encoding service providers.

TUCP secretary general Ernesto Herrera in a statement said the next US administration was doing this to reduce costs and reinforce America’s health care system by having electronic records that could be shared online by hospitals, clinics, doctors, post-acute care facilities and home care agencies.

Job opportunities galore in medical transcription

Hyderabad, Jan. 1 Job cuts, perks slash, cap on expansion are now common trends in the Indian IT/ITES sectors, reeling under the impact of the recession in the US.

However, there seems to be one segment, which is bucking this trend — medical transcription (MT). In fact, there are jobs to be grabbed, say some of the big companies.

Transdyne, among the top 10 MT companies in India, for example plans to add 300-400 staff in 2009. At present, the company has 1,200 employees, mostly in Hyderabad and a few in Vijayawada.

“Our optimism is based on the feeling that a big chunk of business was waiting to be moved to India from the US, which is the biggest source for the Indian MT industry,” said Mr Raghu Vasu, CEO of the company.

The Indian model for providing outsourced MT work is proven in the US. Also, since healthcare is reasonably recession proof, big budget cuts are not expected and hence contracts should be coming to competent Indian companies, he told Business Line.

Bangalore, Hyderabad and Delhi are the main centres for MT, with about a dozen companies being bigger than 1,000 employees.

Medical Transcription Industry - Looking Out for Scams

Medical transcription is a very popular option for working at home. Landing that first job after training can be tough, but it's legitimate work once you get going.

But that's the challenge. Getting things going.

The first thing you need to understand about medical transcription is that you need training at the very least, and quite possibly some experience, before you can find that transcription job from home. Most employers will not train you on the job. Due to the sensitive nature of this field, they need people who are prepared to work and dedicated to their jobs.

Before you sign up with any online medical transcription school, you need to research them. You want to ensure that you have a good shot at actually landing a job afterward. Choose a poor quality school and you are less likely to land any job at all.

There are three schools that are commonly recommended by just about any MT discussion board. They are The Andrews School, M-Tec and CareerStep. The first two are considered by far to be the best, but many employers do approve of CareerStep as well. It's also more affordable than the other two, which is a consideration for many potential students.

That good reputation is key to getting a job in this industry. Without it you are going to struggle to find work. Good schools have a solid reputation, and many of them use the HPI training program. They may also offer job placement assistance.

Do not expect to pay just a few hundred dollars for training. Quality training is a bit pricey, and good programs run at least $1000 and often over $2000. This is not a cheap deal.

But what about community colleges, you may ask? Some do indeed have good programs for less, but they generally lack the reputation that helps you to land that ever so hard to find first job. Talk to the school in detail about their placement rate after training if this is the option you would prefer to pursue.

Watch out for companies that claim to give you training with no experience that will lead to a job. There is one company in particular which changes names regularly, which emphasizes its "Christian" character, yet is nothing more than a scam. They require students to purchase software, and then you never get good enough to get paid. The name change can make them hard to identify, but the routine stays more or less the same.

When it comes time to look for actual work, many employers will consider you to be an independent contractor rather than an employee. This is an important difference both at tax time and in how you get your equipment. You may be expected to provide your own, although some companies may send it to you.

As with any work at home job, employers should not expect you to pay for the privilege of applying to work for them. No fees to show that you're serious, for example. They should be clear about how you will get paid. In most cases you will be paid on production rather than hourly, typically a set rate per line. One line is often 65-75 characters in length.

As with any work at home opportunity, medical transcription scams are out there. But if you are careful and do your research, you can avoid these and enjoy a fruitful career.

Why Not a Less Expensive, Faster Program?

A two-year course is the norm, and the Association for Healthcare Documentation Integrity (AHDI) also recommends that this include a 240 hour externship in an actual healthcare facility. Medical Transcriptionists must sometimes edit the words of the dictating physician and make corrections where appropriate. Courses in English grammar and punctuation, anatomy, physiology, terminology, and medical transcription are necessary. The AHDI recommends that the following course topics are included as part of your training: English grammar and punctuation, medical language, anatomy and physiology, disease processes, pharmacology and laboratory medicine, transcription technology, medical transcription practice, healthcare records, privacy, ethics, and other medicolegal issues

In addition, an effective course of study should be taught by qualified professionals, preferably Certified Medical Transcriptionists (CMTs) who are knowledgeable in their field. The ADHI recommends that at least 30 hours of real dictated physician reports be included in the formal training, plus an additional 10 hours of real dictated physician reports be included in the external practice module.

What about certification?
Watch out for schools that offer a "certificate". It should be clear that this is a certificate of completion of their course of study, NOT the designation "Certified Medical Transcriptionist". Many people are confused as to why transcription (audio typing) apparently costs more than copy typing, as both are typing services. This article aims to explain the differences between the two services and what makes up the costs for a transcription service.

Firstly let's take copy typing. This normally refers to the typing of written 'copy' - this may be a previously typed document (perhaps typed on a typewriter and needing to be input into a Word Processor) or a hand-written document. The cost will vary depending on the ease of readability but with basic copy typing there is no editing or interpretation required; the typist simply types what is written in front of them. The Philippine medical transcription industry will not be spared from the effects of the global economic slowdown although its share in the global market is small, an executive of a transcription company said Tuesday.

Speaking in one of the sessions at the BPO Summit Philippines 2008, Fred Kumetz, chief executive officer of medical and transcription firm eData Services said that the medical transcription (MT) sector is not immune to the effects of the slowdown in the economy of the United States, where most MT clients are based,

“The belief that medical transcription is recession proof is a fallacy,” said Kumetz.

Kumetz, however, said that the effect of the slowdown in the global MT industry will be “totally insignificant” because the Philippine MT industry is too small to be affected by the financial crisis.

The transcription sector is one of the non-voice sectors of the BPO in the Philippines. In 2007, the sector generated US$197 million and it has grown by an average of 24 percent during the past three years.

Medical Transcription Tips for Word Searching

Word searching is locating the medical word that is correct both in spelling and meaning. This is a Medical Transcription skill that takes time and practice to develop.

Tip #1:

If you find yourself unable to locate a medical word in your reference books, it might be the word has an initial letter different than the sound or initial letter you hear. For example, the v sounds very much like that of f, the letter m may actually be an x (xiphoid) and the k might be ch (ischemic).

In addition, medical terms that contain silent letters such as euthyroid, herniorrhaphy, pneumonia or words frequently mispronounced such as menstruation present a unique challenge for the Medical Transcriptionist.

Experience is often more the teacher in using available resources. The longer you remain a Medical Transcriptionist, the more proficient you will become in determining sound-alikes and locating terms with silent letters.

Tip #2:

When you are unable to locate a phrase under a particular entry, look under a related entry.

Tip #3:

When you hear a phrase you cannot understand, look first under the noun and not the adjective. Nouns usually follow adjectives. An example: If you are looking for bullous emphysema, look under emphysema (the noun) and not bullous. To find particular diseases such as Parkinson’s disease, look under disease.

An exception to the above tip is that in Latin phrases, the noun is followed by the adjective such as “fascia lata,” and “ligamentum flavum.” You would look under the first word with Latin phrases. Bacterial names are another exception in that the genus is given first, followed by the species: Neisseria gonorrhoeae.

If you exhaust all you know to do when word searching on the job, a Medical Transcriptionist should:

seek another transcriptionist’s opinion
refer to the patient’s chart
contact the dictating physician for clarification
leave a blank in the report and attach a note so that the dictator may insert the correct word

If you leave a blank in the report, you should flag the report by attaching a note with a paperclip. The note should list all blanks left in the report, and giving as many details as possible for ease of identification such as page and line number. The Medical Transcriptionist may also give a phonetic spelling of what the word or words sounded like to assist the dictator in filling in the blank or blanks.

The main thing to remember about word searching is to never guess at a word. A blank in the medical report does not reflect poorly on the Medical Transcriptionist who thoroughly researched for the words and exhausted all else before leaving the blank. Leaving a blank is the correct thing to do. Integrity and accuracy of the medical report is much more important than presenting documents that never leave blanks.

Reduce ambient room noise

The quieter the room is, the better it will be for the recording. Tips

Remember where the image controls are
The control buttons for zooming in and out and panning are located just above the image. Unfortunately, when the scrollbars are used to scroll down the image the controls are scrolled out of sight. Remember that when you get to the bottom of the image, and want to view additional lines, that you need to scroll back to the top to use the controls. NOTE! the Archives have changed the placement of the image controls to be centered above the image; on low resolution screens you may have to scroll to the right to bring them into view.

Use the tab key!
The tab key can be used to move from input field to input field. The first time you press tab it moves to the first input field, scrolling the window if necessary. This makes it easy to get to the form on a web page without having to use the scrollbar. It also is a much quicker way to move from field to field than having to use the mouse. When the page is first brought up, or if you have clicked in the top part of the window, you need to click in the lower part of the window before using the tab key.

Type the first letter of the value you want in a drop-down selection box
If you use the tab key to select an input field with a drop-down selection box you can select the value you want by simply typing the first letter of the value. If there are multiple values starting with the same letter, press that letter multiple times, the value will cycle through the possible values. For example, in the relation to head of household field, pressing 's' once will select "Son", pressing 's' a second time will select "Sister", and pressing it a third time will select "Servant". If you continue to press a letter it will cycle through the values over and over. Using the tab key and typing the first letter of your choice will generally be much faster than using the mouse to move between fields and select values in drop-down boxes.

A magnifying glass or virtual magnifying glass is sometimes quicker than zooming in on the image
If you just want to take a closer look at a word or name you may find it is quicker to use a magnifying glass or a program like Virtual Magnifying Glass than to reload the image at a higher magnification. On Windows XP there is a built-in magnifier intended for people with visual impairments that can be useful: start / Programs / Accessories / Accessibility / Magnifier

Entering a non-date value in the date of birth column
If the enumerator wrote something like "didn't know" in the month and date of birth column you can enter it by selecting "Other" in the month selection box and then entering the text in the day input box. If the enumerator entered a valid month but then something invalid for the day, e.g. May ? still select "Other" for the month and enter both the month and whatever else in the date column.

Deleting a duplicate line
If you have a duplicate line on a full page edit that line and set the line number to 99. Records with line set to 99 will be automatically deleted periodically. If you have a duplicate line on a page that is not yet complete you can also edit the line and change all the values to data from an as yet untranscribed line. To edit a line use the transcription page and click on the edit link next to the line in the table under the input form. Note: once a page is claimed the edit link will only appear when you are signed on/authenticated as the user that claimed the page.

1911: in the 1911 census you can delete an unwanted line by clicking on the Show Data link below the input form and then clicking on delete next to the line you want to delete.

Notes and corrections
You may edit or add notes to each line previously transcribed. As soon as a line transcription has been submitted, it is available for consultation, editing and/or annotation. Scroll down to the bottom of your screen to access the lines previously transcribed. Have a quick look to make sure there are no duplicate or missing lines. If you find errors in your transcription, you may correct them by clicking on the "Edit" button found at the end of each line. Do the necessary corrections and press Enter or click the "Submit" button, when you have completed editing each line.
Notes:
There are two types of Notes available: surname notes and line notes. It is important to remember that they serve different purposes and should be used accordingly.

Surname Notes are used to offer an alternative surname to the one listed on the census where the enumerator has made a spelling mistake in a name. It should always be attached to the line of the head of the family. A Surname Note affects a whole family, not only the individual listed on the line to which it is attached. Furthermore, whenever an alternative surname is suggested in a Surname Note, it will also be included in the National Surname Index with a link to the related family. Please remember that Surname Note cannot be edited or deleted. So they should be done with care!

Line Notes, on the other hand, are used to add a comment when a line is struck out, contains comments or other information not easily captured in the standard fields. Line Notes are attached to a line number, not to the individual listed on that line. Please remember that if you change a line number, the note will remain attached to the line number originally affected by the note. For example, line number 20 has a note attached, correcting the field known as "relation to the head of the family" for a certain individual. When reviewing your work, you realize that line 20 should actually be line 21; using the "Edit" button, you correct the field "line no." from #20 to #21 and click "Submit". The individual previously listed on line 20 is now appearing on line 21, as it should be, but the line note remained with line #20. Therefore, your Note is now attached to the wrong individual. You have no other option but to delete the Note attached to line 20, and to do a new one, this time, on line 21. You may use the copy/paste commands to save yourself some time. Always remember to delete Notes which are no longer needed, specially when correcting a line # to 99, as the Note will remained attached to the original line number. To view the complete list of all the line notes added to one page, first access the transcription page, after you are logged in, and scroll down to the bottom part of your screen, right under the transcription board. The delete option can be found under each line note.

1911: to delete a line note you have added in the 1911 index go to the transcription screen, click on Show data, click on the note link for the line the note appears on, and click the delete link next to the note in the list of existing notes for that line.

Surname Note:

Scroll to the bottom part of your screen, to the transcriptions board. Click on "Note" to the right of the line of your choice. The next screen will show the complete line with the information as entered. Under it, you will see "Add surname note". The field next to it contains the family name as it has been entered. You have the option to edit the name to offer an alternative: overwrite it with the alternative surname. If necessary, use the next field to add the appropriate comment to support your alternative, and click on "Submit alternative surname".
Line Note:
In the field "Add line note", include additional comments or other information not easily captured in the standard field. If a line is crossed out or a note appears in the margin in the original you can use a line note to include this information.
If you have records such as birth, baptism, or marriage records that indicate that information given in the census is incorrect, or giving additional useful information, you can add a line note citing that record and giving the alternate information. By using line notes the index stays true to the original census record while the user is alerted to the additional information. The ability to add information should only be used where you can cite a record, the index will become cluttered and confusing if a lot of unsupported information is added. Line notes are limited to about 200 characters so plan carefully in order to enter as much as possible in the space available. If you have important information to share, if necessary, you may add an additional line note to the same line.

Note: line notes should never be used to correct a transcription error! Correction notes should be used for that purpose.

Guidelines

Repeated values
Where the value of a field on the original census form indicates a repeat of the previous value either with a quotes/ditto mark or, in the case of the household number by being left blank, the value should be repeated in the transcription rather than using a ditto mark or leaving the field empty. This makes each record complete in itself which is useful for searching and sorting purposes.

Capitalization
The capitalization used in the original census form should be preserved in the name fields. Surnames should not be entered completely in upper case letters. Where output is desired with the surname in upper case it is trivial for the computer to produce all upper case from a mixed case value but not the other way around.

Errors in the original census
Where the enumerator has made a spelling error in a name or one of the other values seems to be or is in error, the transcription should be faithful to the value entered by the enumerator. Conflicts in the date of birth versus the age are particularly common and should be transcribed as they are written in the original census. For fields with selection boxes (e.g. month) the appropriate value in the box should be selected even if the enumerator has used a different abbreviation or misspelled the word.

Unreadable entries
In some cases the transcriber will not be able to make out what is written in a field. In these cases a question mark (?) can be used to indicate an unreadable value. If a value is partially legible then the part that is legible should be entered and the part that is not should be represented with a question mark. If the transcribers feel they can make a reasonable guess at the characters then they should enter their guess rather than using question marks. For example, if * represents an unreadable character in the original then:

Crossed out lines
If a line is filled in and then crossed out enter the values for the line and use the note link next to the line in the table below the input form to add a line note indicating the line was crossed out. If there was a note on the original form such as "dead" include that in the line note comment.

Blank lines
If a line is left blank all values for that line (except the line number!) should be set to blank. It is preferable for blank lines to be represented as blank rather than apparently missing. On the last page of a polling district there will usually be some blank lines, to fill these in use the link at the bottom of the transcription page which will bring up an alternate transcription form with all the default values set to blank, making it easier to add blank lines, you only need to fill in the line number for each line. If you want to fill in the rest of the page with blank lines there is a link at the top of this alternate transcription form that will fill in blanks from the last transcribed line to the end of the page. If there are blank lines in the middle of the page you will need to fill them in line by line.

Surname and givens order reversed
Some enumerators entered names with the given names first instead of surnames first. Sometimes entire pages were recorded this way, other times a single household. If a whole page is recorded this way or it is otherwise clear which is the surname and which is the given name then they should be entered in the input boxes labeled surname and givens regardless of the order in which they are written in the original. If the correct order for a single household is ambiguous then the names should still be entered according to the order they were written and a surname note should be added. For example, if an entry is written "John Henry" in an area where Henry is a common surname and John is not, then it should be transcribed with surname John and givens Henry but a surname note added with the surname Henry. Adding the surname note flags the entry so that users are alerted to the need to consult the image and judge for themselves, and adds an extra entry to the surname list making it possible to find the person if it turns out that what was written as the givens is actually the surname.

Abbreviated given names and initials
Sometimes the enumerator abbreviated given names, e.g. Geo. for George. Such abbreviations should be transcribed as they appear in the original. Some enumerators put periods after initials, others did not, and some were inconsistant. The preferred transcription is to record exactly what was written in the original but this is not critical and one should not enter corrections where the only difference between the original and the transcription is a period after an initial.

Two digit birth years
Where the enumerator has written birth years as two digit numbers instead of the full four digits the birth year should be expanded to four digits. Like month names that are abbreviated in various ways, the value of the birth year is not being changed when it is written in full and no interpretation is necessary.

Duplicate, missing, and corrected household numbers
The household numbers have no intrinsic meaning, they were just assigned in the order of enumeration. Since the order of enumeration is already reflected in the page and line ordering they carry even less significance (in the past the family number could be useful where the transcription was presented ordered alphabetically by surname of the head of household which is not an issue with our index). The importance of the household number in our index is in grouping people into households, so what the number is is less important than that all the members of a given household have the same, and preferably unique number. Having said that, where the numbers are clear and legible they should be entered as is. Where a number is repeated the onus is then on the user of the index to recognize the anomaly and draw their own conclusions. If two households have the same household number add 1000 to the household number for the second household in order to ensure it is unique. A line note should then be added to explain that this has been done. In the common case where the numbers have been written over it is preferable to choose the numbers that correctly group the households. Often the original numbers have been corrected but the correction is fainter or less clear than the original, it is still preferable to record the fainter but correct numbers or simply use sequential numbers where the overwriting makes the numbers in the original illegible. Note that the combination of page and line number create an unambiguous reference to a person and will normally provide a better means of refering to a record than using a household number both because of how commonly the household number is unclear and because the household number has no real meaning. Use of household numbers for references are mainly an artifact of printed transcriptions that have been reordered and online indices that do not provide listings of whole pages in the original order. In particular, note that refering to a page and line number does not tie your reference to a given index in any way, the page and line numbers correspond to the original documents.

Groups with no surname
There are several cases where a group of people did not use surnames including first nations peoples and religious orders. In these cases we have elected to use an identifying keyword for the missing surname and use the single name as a given name. Some example artificial surnames are: Sister (for nuns), Inuit, Blackfoot. This scheme has the added benefit that it makes it significantly easier to search the index.

Inclusion of titles
In cases where the enumerator has included a title it should be transcribed at the end of the givens field. For example, "Wood, Senator Josiah" should be transcribed surname "Wood" and givens "Josiah, Senator".

Using Amazon Mechanical Turk for transcriptions: Effective, but it's a kluge

Andy Baio has an interesting review of Amazon's Mechanical Turk being used for "cheap, easy" audio transcription. The review immediately caught my eye -- I've been manually transcribing interviews for years, and, as Baio notes, it's a royal pain. Human transcription services are one alternative, but they tend to be expensive and a hassle to arrange. I've been waiting for some company to launch a cheap, automated, accurate transcription service or software package. Could Mechanical Turk be the answer?

After reading the review, I've come to the conclusion that it's not. The service is cheap, but it is definitely not automated -- remember, this is Mechanical Turk, which is a marketplace for buyers and sellers of services that need to be done by human beings.
